I would like to introduce to you a wonderful ministry that I am pleased to be a part of. That ministry is Life Choices Clinic.
Life Choices is a medical clinic that is dedicated to providing resources and information to men and women regarding unplanned pregnancies, sexual health, STD’s, relationships, etc. We are there to not only educate, but to empower our clients to make informed decisions for their future.
All of the services we provide are free, we are a donation based organization. We receive our support from churches, other organizations and people that have a passion for this ministry.
I am currently a volunteer counselor there. I work 4 hour shifts every week in the Kenmore clinic.

One thing I can't say enough to our clients is: "Please check your benefits!" It is not the clinic's responsibility to know your benefits. It is very important that you contact your insurance agent and ask them the following questions:
-Do I have benefits for naturopathic services?
-Do I have benefits for massage and chiropractic services?
-Do I have limitation of services?
-What physicians are contracted?
-What hospitals and labs are contracted?
-Do I have out of network benefits to see a physician outside the network?
Our clinic does not do any lab billing and therefore we are not knowledgable of the pricing of the tests we run. If you would like to know the prices of tests before having your blood drawn, please specifiy this at your appointment and we will make a call for you to obtain that information. The laboratories we work with are: Evergreen Hospital, Northwest Hospital, PAML.

Cancellation policy:
$75 charge for no-show appointments as well as cancellations made in less then 24 hours.
Accepted insurance plans:
Alternare, Uniform Medical Plan, United Healthcare, Cigna, Aetna U.S. Healthcare, Premera Blue Cross, KPS, First Choices, and other plans that have Naturopathic or out of network benefits.

What is your emergency procedure?
If you have a medical emergency, you need to call 911. If you are a patient here and have a health concern that can’t wait until your next appointment time, you may call our answering service at 425-697-6112 and have the physician on-call to be paged. There is an additional $65 dollar charge for this service.
How does your scheduling work?
First office visits are usually about an hour long, and the waiting list is 2 weeks. Return visits are usually about 30 minutes long, and the waiting list is 5-7 days. Emergencies are scheduled within 1-2 days depending on severity. Due to time constraints we require that our patients arrive to their appointment on time. If a patient is 15 minutes late, we will need to reschedule that appointment.
What do I need to know before my first visit?
When coming in for your first visit, you will need to bring in copies of your medical records, as well as lab results performed within the last 6 months. It would be helpful if you could come in 15 minutes early to fill out paperwork.
What products do you use?
Our products are natural and consist of vitamins, herbs, botanicals and various nutrients. We order from companies such as Thorne Research, Integrative Therapeutics, Wise Woman Herbals, Heel Homeopathic, Biogenesis, etc. The products we carry and prescribe have been researched by our physicians for quality and results.
What insurance companies offer the best benefits:
My experience has been that Premera Blue Cross, Aetna U.S. Healthcare and KPS Health Plans appear to offer the best benefits.
What are some insurance matters I need to know?
Many companies offer an out-of network and in-network plan. In other words, if the provider that you want to see is not contracted with the insurance company, they will pay a lower percent for the service provided. Whereas, if the provider you selected is contracted, then they will pay a higher percentage.
There are deductibles on most plans as well. These deductibles can range from $200-$1500 per person, per calendar year. Deductibles are renewed each year and you will not have access to your insurance benefits until they are met. After you have paid your deductible, then your insurance will cover their percentage.
Many insurance plans are no longer covering at 100%. You will find that you may receive a monthly statement from your physician for the balance of the service charge that was not covered. Your patient responsibility will range from 30%-40% depending on your plan.
Another thing to remember, is that referrals may be necessary. In other words, before you go to a specialist, you may need prior authorization from your insurance company first. If this is required, but not obtained, you are subject to paying the entire fee for your visits with the specialist.
Pharmacy items, and some lab testing are not covered by insurance companies. You will need to check with your insurance company to obtain those benefits.
Some insurance companies set limitations on services such as naturopathic, massage and chiropractic.
